How physios use a TENS machine

Experiencing chronic pain, or discomfort from an injury that doesn’t seem to be improving using only manual therapies? Then using a TENS machine might be the next step in your treatment journey.

A TENS machine is a type of electrotherapy treatment. Also referred to as Transcutaneous Electrical Nerve Stimulation, it is commonly used as a pain relief treatment, but it has a range of applications, including being used by women in labour for a reprieve of pain, through to stimulating muscle response and toning the body.

As physios, we most commonly incorporate a TENS machines into a personalised treatment plan for one of three main reasons; to block pain signals, to relieve tension or to stimulate a muscle that isn’t responding to messages from the brain

 

A TENS machine works by sending a small electrical current from the targeted area to the brain. Electrodes (pads) from the machine are applied to the skin of the area concerned, and a level of current and intensity is then selected by your therapist in order to effectively treat the problem at hand. Essentially, the electrical currents will help reduce the sensation of pain by blocking the message from the problem area, provoking the release of endorphins and/or relaxing the muscle of focus

What can you commonly treat using a TENS machine?

TENS machines can be used to treat a number of ailments such as knee pain, neck pain, back pain, sciatica and sporting injuries. As a pain relief treatment, the intensity and level of the current will result in either a relaxing of the muscle to find relief, or for the stimulation of endorphin release.

Another use for a TENS machine is to build muscle tone and boost athletic performance. When being used for muscle activation therapy such as this, our team of specialists advise for the device to be applied whilst you are doing your allocated physio exercises at the gym. This way muscles are stimulated and at their peak for your workout.

 

Can a TENS machine be harmful?

TENS machines are considered safe, but like all treatments there are some considerations, which your therapist will take into account when opting to use the device. While a TENS machine is a great alternative to those who are wanting to see results without the use of needles or medications, it shouldn’t be used in the following instances:

  • On irritated skin or open wounds.
  • Near the eyes, or other sensitive areas.
  • While operating machines, such as driving.
  • While near or in water.
  • Pregnant women who are not in labour.
  • People with a pacemaker or a cochlear implant.
  • People with epilepsy.

A TENS machine can be found to help a variety of musculoskeletal (MSK) conditions, such as nerve pain, muscle pain, osteoarthritis, sports injuries, menstrual pain, back pain, migraines and more.
 

Why Use TENS machine for labour (pregnancies)?

 
Just as each pregnancy is different, the effects of a TENS machine can differ from one woman to another during labour. The electrodes are placed on the mother’s lower back, and the electric stimulations are controlled throughout the contractions.
 
It is not confirmed whether TENS is used commonly for labour because of the amount of pain that is reduced, or due to the increased feeling of control felt during the process.

Sometimes TENS is used after surgery for incision or post-operative pain (e.g., joint replacement, cardiac procedures, various abdominal surgeries, or cesarean sections). Studies show that TENS can significantly reduce the use of analgesics (pain relievers, including narcotic drugs) after surgery.

TENS is usually used along with other forms of treatment and pain control such as analgesics, relaxation therapy, biofeedback,

HOW DOES A TENS MACHINE WORK

TENS machines produce an electrical impulse that can be adjusted for pulse, frequency, and intensity. The exact mechanism by which it works to reduce or even eliminate pain is still unknown.  There are several possible ways TENS works. For example, TENS may inhibit (block) pain pathways or increase the secretion of the pain-reducing substances (e.g., endorphins, serotonin) in the central nervous system.

Recent research has also shown that autosuggestion or the placebo effect is a powerful way many people experience pain relief or decrease their symptoms.  This means that by simply believing a treatment it has a beneficial effect on the nervous system. This effect can be true for the positive effects of using TENS as well.

 

CONTRAINDICATIONS TO USING TENS (WHEN YOU SHOULD NOT USE TENS)

  • If you have a loss of skin sensation or even decreased sensation. With altered sensation, there is a risk of turning the unit up too high and causing an injury.

  • If you have a cardiac pacemaker as the electrical signals could interfere with the pacemaker. Cardiac patients should not use TENS without their physician’s approval.

  • If you have Alzheimer’s Disease, dementia, or other cognitive problems as the unit may not be used properly and can cause injury.

Transcutaneous Electrical Nerve Stimulation (TENS)

TENS is widely used around the world for a variety of painful conditions such as;

  • Arthritis

  • Low back pain

  • Labour pain

  • Nerve related pain such as phantom pain

How Does It Work?

A small electrical device (think back to the days of the Walkman!) delivers electrical impulses across the skin.  Lightweight versions not much bigger than a credit card are available these days. This can be clipped to your jeans or placed in your pocket.

The device is connected by wires to sticky pad electrodes, which are placed on the skin in the area of the pain. This allows a small, low-intensity electric charge to be passed across the area.

 

1/ On a high frequency, by selectively stimulating certain ‘non-pain’ nerve fibres to send signals to the brain that block other nerve signals carrying pain messages. High frequency stimulation, sometimes called “conventional”, is tolerable for hours, but the resultant pain relief lasts for a shorter period of time.

2/ Lower frequencies stimulate the production of endorphins, natural pain-relieving hormones – your own built-in pain management system.  Low-frequency stimulation, sometimes called “acupuncture-like”, is more uncomfortable and tolerable for around 20-30 minutes, but the resultant pain relief lasts longer.

Unlike many pain-relieving drugs, TENS isn’t addictive and has few side-effects. Most people can use a TENS machine but it is unsuitable for:

-Epileptics.
-Patients with pacemakers and certain other types of heart disease.
-Unknown causes of pain.
-Certain body sites in pregnancy (other than in labour)
-Certain skin conditions

TENS users should experiment with various electrode placements. Electrodes can be placed over the painful area, surrounding the painful area, over the nerve supplying the painful area, or even on the opposite side of the body. TENS users need to try the unit for several days with several electrode placements prior to deciding if it will be useful. A home trial for several days to weeks is preferable.
