Transcutaneous Electrical Nerve Stimulation (TENS) 

One of the most common electrotherapy treatments, Transcutaneous Electrical Nerve Stimulation (TENS) makes use of small devices which deliver pulses of electrical stimulation. The goal of the TENS machine is to stimulate sensory nerves to achieve pain relief and is often used for both nerve pain and chronic pain conditions.

 

TENS is a non-invasive, safe nerve stimulation treatment intended to reduce pain, both acute and chronic.  There is a small electrical device that delivers electrical impulses across the skin. The device is usually about the size of a personal stereo, although lightweight versions not much bigger than a credit card are available.
The device is connected by wires to sticky pad electrodes, which are placed on the skin in the area of the pain. This allows a small, low-intensity electric charge to be passed across the area.
How do 'TENS' devices work? Transcutaneous means 'through the skin'. TENS devices deliver small electrical pulses to the body via electrodes placed on the skin. TENS devices are thought to affect the way pain signals are sent to the brain. Pain signals reach the brain via nerves and the spinal cord. If pain signals can be blocked then the brain will receive fewer signals from the source of the pain. We may then feel less pain.

 

 

Unlike many pain-relieving drugs, TENS isn't addictive and seems to have few side-effects. Most people can use a TENS device but it's not suitable for:

  • Epileptics.
  • Those with pacemakers and certain other types of heart disease.
  • Unknown causes of pain.
  • Unsupervised in pregnancy (other than in labour).

 

TENS is widely used around the world for a variety of painful conditions (for example, there are an estimated 450,000 users annually in Canadian state hospitals, while in Scandinavia TENS is well known as a treatment for period pain). In the UK, the use of TENS is increasing in many fields and, whilst originally used by women in labour, it's now marketed for most sorts of pain, especially musculo-skeletal.

 


TENS by definition covers the complete range of transcutaneously applied currents used for nerve excitation although the term is often used with a more restrictive intent, namely to describe the kind of pulses produced by portable stimulators used to treat pain. The unit is usually connected to the skin using two or more electrodes. A typical battery-operated TENS unit is able to modulate pulse width, frequency and intensity. Generally TENS is applied at high frequency (>50 Hz) with an intensity below motor contraction (sensory intensity) or low frequency (<10 Hz) with an intensity that produces motor contraction.

While controversy exists as to its effectiveness in the treatment of chronic pain, a number of systematic reviews or meta-analyses have confirmed its effectiveness for postoperative pain, osteoarthritis, and chronic musculoskeletal pain.

 

Conversely, results from the Bone and Joint Decade 2000-2010 Task Force on Neck Pain, reported in early 2008, showed no clinically significant benefit to TENS for the treatment of neck pain when compared to sham treatment.

 

TENS electrodes should never be placed:

  • Over the eyes due to the risk of increasing intraocular pressure
  • Transcerebrally
  • On the front of the neck due to the risk of a acute hypotension (through a vasovagal reflex) or even a laryngospasm
  • Through the chest using an anterior and posterior electrode positions, or other transthoracic applications understood as "across a thoracic diameter"; this does not preclude coplanar applications
  • Internally, except for specific applications of dental, vaginal, and anal stimulation that employ specialized TENS units
  • On broken skin areas or wounds, although it can be placed around wounds.
  • Over a tumour/malignancy (based on in vitro experiments where electricity promotes cell growth)
  • Directly over the spinal column

 

TENS should not be used across an artificial cardiac pacemaker (or other indwelling stimulator, including across its leads) due to risk of interference and failure of the implanted device. Serious accidents have been recorded in cases when this principle was not observed. 

 

TENS APPLICATION

Health care providers have been using electricity to help relieve pain since the mid-1960s. This form of pain relief sends electricity directly to parts of the body. It's called transcutaneous electrical nerve stimulation, or TENS. A growing body of research says that TENS therapy may help some people in the right situation.

Understanding nerve stimulation therapy

The main reason that people use electric nerve stimulation therapy is to ease pain. Some of the common conditions that cause pain and may be helped by TENS include arthritis, tendonitis, bursitis, surgery, migraines or other headaches, wounds, and injuries. Some experts believe electric nerve stimulation therapy has the greatest use for treating nerve pain.

Nerve stimulation therapy delivers mild electrical impulses to the body. Research on electric nerve stimulation

Hundreds of studies have been done on electric nerve stimulation, but most have been small or weren't well designed. For this reason, some experts say TENS can give short-term relief. Long-term relief hasn't been proved.

More research is needed to know if TENS can help many kinds of acute or chronic pain. 

For cancer pain, more research is needed before TENS can be recommended as a treatment for cancer pain. TENS may give short-term relief for some nerve-related cancer pain, according to the American Cancer Society.

The safety of electric nerve stimulation

Electric nerve stimulation is generally regarded as safe. But it has risks just like any other medical procedure. For example, if the electrical current is too high or the electrodes are placed on the wrong part of the body, this can burn or irritate the skin. The "danger zones" include the brain, heart, eyes, and throat. In addition, people with heart problems, pregnancy, or implanted devices, including infusion pumps, pacemakers, and defibrillators, should not get this treatment.

Transcutaneous Electrical Nerve Stimulation

Transcutaneous electrical nerve stimulation (TENS) currently is one of the most commonly used forms of electroanalgesia. Hundreds of clinical reports exist concerning the use of TENS for various types of conditions, such as low back pain (LBP), myofascial and arthritic pain, sympathetically mediated pain, bladder incontinenceneurogenic pain, visceral pain, and postsurgical pain. Because many of these studies were uncontrolled, there has been ongoing debate about the degree to which TENS is more effective than placebo in reducing pain.

 

Transcutaneous Electrical Nerve Stimulation (TENS)

Introduction

TENS is a method of electrical stimulation which primarily aims to provide a degree of symptomatic pain relief by exciting sensory nerves and thereby stimulating either the pain gate mechanism and/or the opioid system. The different methods of applying TENS relate to these different physiological mechanisms. The effectiveness of TENS varies with the clinical pain being treated, but research would suggest that when used ‘well’ it provides significantly greater pain relief than a placebo intervention. There is an extensive research base for TENS in both the clinical and laboratory settings and whilst this summary does not provide a full review of the literature, the key papers are referenced. It is worth noting that the term TENS could represent the use of ANY electrical stimulation using skin surface electrodes which has the intention of stimulating nerves. In the clinical context, it is most commonly assumed to refer to the use of electrical stimulation with the specific intention of providing symptomatic pain relief. If you do a literature search on the term TENS, do not be surprised if you come across a whole lot of ‘other’ types of stimulation which technically fall into this grouping.

TENS is most commonly delivered from small, hand held, battery powered devices. They can be purchased 'over the counter' in many (but not all) countries. In some locations, they need to be 'prescribed' by a therapist, doctor or other healthcare practitioner. Most multi-modal clinic based stimulators include TENS as an option, though its use in the clinic is less well supported than its use as a home based, patient delivered therapy. Examples of typical TENS units are illustrated below.

CONTRAINDICATIONS   

Patients who do not comprehend the physiotherapist’s instructions or who are unable to co-operate
It has been widely cited that application of the electrodes over the trunk, abdomen or pelvis during pregnancy is contraindicated BUT a recent review  suggests that although not an ideal (first line) treatment option, application of TENS around the trunk during pregnancy can be safely applied, and no detrimental effects have been reported in the literature 
TENS during labour for pain relief is both safe and effective
Patients with a Pacemaker should not be routinely treated with TENS though under carefully controlled conditions it can be safely applied. It is suggested that routine application of TENS for a patient with a pacemaker or any other implanted electronic device should be considered a contraindication.
Patients who have an allergic response to the electrodes, gel or tape
Electrode placement over dermatological lesions e.g. dermatitis, eczema
Application over the anterior aspect of the neck or carotid sinus

PRECAUTIONS

If there is abnormal skin sensation, the electrodes should preferably be positioned elsewhere to ensure effective stimulation
Electrodes should not be placed over the eyes
Patients who have epilepsy should be treated at the discretion of the therapist in consultation with the appropriate medical practitioner as there have been anecdotal reports of adverse outcomes, most especially (but not exclusively) associated with treatments to the neck and upper thoracic areas
Avoid active epiphyseal regions in children (though there is no direct evidence of adverse effect)
The use of abdominal electrodes during labour may interfere with foetal monitoring equipment and is therefore best avoided
