Transcutaneous Electrical Nerve Stimulation (TENS)

A TENS unit has controls that allow people to administer an appropriate level of pain relief. People can achieve this by altering the following aspects of the electrical current:

Intensity: A dial allows the user to adjust the intensity of the electrical stimulation.

Frequency: The frequency refers to the number of electrical pulses per second. High-frequency (HF) pulses range from 80 to 120 cycles per second and may help manage acute pain. Low-frequency (LF) pulses range from 1 to 20 cycles per second and are suitable for the treatment of chronic pain.

Duration: The duration is the number of microseconds that the current enters the skin for during each pulse.

 

 

Transcutaneous Electrical Nerve Stimulation (TENS)

Electrical nerve stimulation is a treatment for pain that can be used for acute pain (e.g., during labor or after surgery) or for chronic pain (e.g., low back pain). It is a form of electrical energy sent in various waveforms to the nerves. When it is delivered through electrodes or patches placed on the skin, it is called transcutaneous (meaning through the skin) electrical nerve stimulation or TENS for short.

TENS is a noninvasive way to override or block signals from the nerves to the spinal cord and brain. Pain messages may be altered enough to provide temporary or even long-lasting pain relief. In addition to controlling pain, this type of electrical stimulation can also improve local circulation and reduce or eliminate muscle spasm.

 

 

Who May Benefit from a TENS

TENS can be used for relief of pain associated with a wide variety of painful conditions.  Back pain caused by spine degeneration, disc problems, or failed back surgery is commonly relieved by the use of TENS.  Nerve pain (neuropathy) from conditions such as chronic regional pain syndrome (CRPS) or diabetes may also be managed with TENS.

TENS has been used for people suffering from cancer-related pain, phantom-limb pain (a chronic pain syndrome following limb amputation), and migraine or chronic tension-type headaches.

TENS can also be used for muscle soreness from overuse, inflammatory conditions, and both rheumatoid and osteo-arthritis. Athletes with painful acute soft tissue injuries (e.g., sprains and strains) may benefit from TENS treatment.

Sometimes TENS is used after surgery for incision or post-operative pain (e.g., joint replacement, cardiac procedures, various abdominal surgeries, or cesarean sections). Studies show that TENS can significantly reduce the use of analgesics (pain relievers, including narcotic drugs) after surgery.

TENS is usually used along with other forms of treatment and pain control such as analgesics, relaxation therapy, biofeedback, visualization or guided imagery, Physical Therapy, exercise, massage therapy, nerve block injections, and/or spinal manipulation.

How does a TENS machine work

TENS machines produce an electrical impulse that can be adjusted for pulse, frequency, and intensity. The exact mechanism by which it works to reduce or even eliminate pain is still unknown.  There are several possible ways TENS works. For example, TENS may inhibit (block) pain pathways or increase the secretion of the pain-reducing substances (e.g., endorphins, serotonin) in the central nervous system.

What Is TENS and How Does It Work?

“Transcutaneous electrical nerve stimulation is the application of electrical current through the skin for pain relief,” says researcher Kathleen Sluka, PT, PhD, a professor of physical therapy and rehabilitation science at the University of Iowa.

The TENS device has been around for decades, and although it is not known exactly how the stimulation works, there are theories. “TENS activates our body’s own painkillers,” says Dr. Sluka, who has extensively studied TENS. “Specifically, it can release our natural opioids, endorphins, and enkephalins [another naturally occurring brain compound], and other chemicals to stop pain signals in the brain.”

Activating these chemicals, Dr. Sluka says, reduces pain signals in the central nervous system.

In addition, “TENS works best for activity-induced pain and should therefore be used when doing activities such as walking, chores, or exercise,” she says. “TENS is used as a method to control pain to allow people to be more active.”

 

 

How does a TENS machine work?

Transcutaneous Electrical Nerve Stimulation (TENS) uses electrodes to stimulate sensory or motor nerve endings in a therapeutic fashion. TENS is also used to block pain signals being sent to the brain. Over time, this can help retrain damaged nerves, resulting in tremendous rehabilitative effects.

Using a TENS machine also naturally enhances the flow of endorphins to the brain. Studies have also shown that using a TENS machine regularly decreases inflammation. This therapy has been found to be useful with patients who have conditions such as:

  • Chronic back, neck, or shoulder pain;
  • Nagging injuries;
  • Pinched nerves;
  • Sciatica;
  • Vertebral disc degeneration.

Despite a scary-sounding name, it may surprise you to learn that using a TENS machine is virtually painless. Patients report some tingling sensation or minor involuntary muscle spasms. The degree of these effects is related to how strong a current the TENS machine is set to. This machine can be used in a chiropractic office with a professional or at home, provided you have formal training in its use.

 

 

What is a TENS unit?

TENS stands for transcutaneous electrical nerve stimulation. Do not let the name scare you. An electronic TENS unit is simple to use and has worked well to control certain kinds of pain. Although it is not a cure, it is a routine part of pain management.

How does it work?

A TENS unit is a small, battery-operated device about the size of a beeper. It sends a small electric signal through self-stick pads that are placed in the painful area. The signal passes through the skin to the nerve fibers and stops pain signals before they reach the brain. If these signals do not reach the brain, then no pain is felt.

There are several theories about how a TENS unit works. The “gate control” theory suggests that TENS signals reach the pain centers in the brain before pain signals do. Like a busy signal on a telephone, the TENS closes the “gate” so that pain signals cannot get through. A second theory suggests that the short, mild electrical signals from the TENS unit cause the body to release its own pain relievers, called endorphins.

 

Contraindications to using TENS (When you should NOT use TENS)

  • If you have a loss of skin sensation or even decreased sensation. With altered sensation, there is a risk of turning the unit up too high and causing an injury.

  • If you have a cardiac pacemaker as the electrical signals could interfere with the pacemaker. Cardiac patients should not use TENS without their physician’s approval.

  • If you have Alzheimer’s Disease, dementia, or other cognitive problems as the unit may not be used properly and can cause injury.

 

Contraindications

  • Patients who do not comprehend the physiotherapist’s instructions or who are unable to co-operate
  • It has been widely cited that application of the electrodes over the trunk, abdomen or pelvis during pregnancy is contraindicated BUT a recent review suggests that although not an ideal (first line) treatment option, application of TENS around the trunk during pregnancy can be safely applied, and no detrimental effects have been reported in the literature (see www.electrotherapy,org for publication details).TENS during labour for pain relief is both safe and effective.
  • Patients with a Pacemaker should not be routinely treated with TENS though under carefully controlled conditions it can be safely applied. It is suggested that routine application of TENS for a patient with a pacemaker or any other implanted electronic device should be considered a contraindication.
  • Patients who have an allergic response to the electrodes, gel or tape
  • Electrode placement over dermatological lesions e.g. dermatitis, eczema
  • Application over the anterior aspect of the neck or carotid sinus[4]

 

Precautions

  • If there is abnormal skin sensation, the electrodes should preferably be positioned elsewhere to ensure effective stimulation
  • Electrodes should not be placed over the eyes
  • Patients who have epilepsy should be treated at the discretion of the therapist in consultation with the appropriate medical practitioner as there have been anecdotal reports of adverse outcomes, most especially (but not exclusively) associated with treatments to the neck and upper thoracic areas
  • Avoid active epiphyseal regions in children (though there is no direct evidence of adverse effect)
  • The use of abdominal electrodes during labour may interfere with fetal monitoring equipment and is therefore best avoided.
