IFT ( Interferential Therapy )
Introduction & IFT Production
The basic principle of Interferential Therapy (IFT) is to utilise the strong physiologicaportable iftl effects of low frequency (<250pps) electrical stimulation of nerves without the associated painful and somewhat unpleasant side effects sometimes associated with low frequency stim.
To produce low frequency effects at sufficient intensity at depth, patients can experience considerable discomfort in the superficial tissues (i.e. the skin). This is due to the impedance of the skin being inversely proportional to the frequency of the stimulation. In other words, the lower the stimulation frequency, the greater the impedance to the passage of the current & so, more discomfort is experienced as the current is ‘pushed’ into the tissues against this barrier. The skin impedance at 50Hz is approximately 3200* whilst at 4000Hz it is reduced to approximately 40*. The result of applying a higher frequency is that it will pass more easily through the skin, requiring less electrical energy input to reach the deeper tissues & giving rise to less discomfort.
The effects of tissue stimulation with these 'medium frequency' currents (medium frequency in electromedical terms is usually considered to be 1KHz-100KHz) has yet to be established. It is unlikely to do nothing at all, but in terms of current practice, little is known of its physiological effects. It is not capable of direct stimulation of nerve in the common context of such stimulation.
Interferential therapy utilises two of these medium frequency currents, passed through the tissues simultaneously, where they are set up so that their paths cross & they literally interfere with each other. This interference gives rise to an interference (beat frequency) which has the characteristics of low frequency stimulation – in effect the interference mimics a low frequency stimulation.
The exact frequency of the resultant beat frequency can be controlled by the input frequencies. If for example, one current was at 4000Hz and its companion current at 3900Hz, the resultant beat frequency would be at 100Hz, carried on a medium frequency 3950Hz amplitude modulated current.
Physiological Effects & Clinical Applications
It has been suggested that IFT works in a ‘special way’ because it is ‘interferential’ as opposed to ‘normal’ stimulation. The evidence for this special effect is lacking and it is most likely that IFT is just another means by which peripheral nerves can be stimulated. It is rather a generic means of stimulation – the machine can be set up to act more like a TENS type device or can be set up to behave more like a muscle stimulator – by adjusting the stimulating (beat) frequency. It is often regarded (by patients) to be more acceptable as it generates less discomfort than some other forms of electrical stimulation.
The clinical application of IFT therapy is based on peripheral nerve stimulation (frequency) data, though it is important to note that much of this information has been generated from research with other modalities, and its transfer to IFT is assumed rather than proven. There is a lack of IFT specific research compared with other modalities (e.g. TENS).
Selection of a wide frequency sweeps has been considered less efficient than a smaller selective range in that by treating with a frequency range of say 1-100Hz, the effective treatment frequencies can be covered, but only for a relatively small percentage of the total treatment time. Additionally, some parts of the range might be counterproductive for the primary aims of the treatment.

What is interferential therapy?
Interferential therapy (IFT) is a form of electrical stimulation that is used to treat muscular spasms and pain and accelerate healing after an injury.
During IFT, electrodes are placed on the skin above the affected region. A mild electrical current is passed through the skin into the muscle tissue, causing it to contract.
IFT is a pain-free procedure, though some patients experience a mild tingling or tickling sensation on their skin.
How does interferential therapy work?
IFT uses low-frequency electrical currents to stimulate nerve tissues, which cause muscles to contract. The main difference between IFT and conventional stimulation techniques is that IFT uses two medium-frequency currents that intercept and "interfere" with one another. This interference creates a low-frequency stimulation. Conventional stimulation uses low-frequency currents directly.
Because IFT uses two medium-frequency electrodes on the skin to create a low-frequency current in the tissue, it is reported to be more tolerable than conventional stimulation that uses low-frequency currents on the skin.
It is currently unclear if IFT offers clinical advantages (e.g. in terms of pain relief) over conventional stimulation, rather it is another method of offering low-frequency stimulation to peripheral nerves.
Interferential Therapy

What is a TENS machine?

While using a TENS machine, you'll experience a non-painful tingling or buzzing sensation, and this can help to block or suppress pain messages. Two electrode pads are usually positioned either side of the area of most intense pain. This is done so that the TENS sensation covers the painful area.
What is transcutaneous electrical nerve stimulation (TENS) therapy?
Transcutaneous electrical nerve stimulation (TENS) is a therapy that uses low voltage electrical current to provide pain relief. A TENS unit consists of a battery-powered device that delivers electrical impulses through electrodes placed on the surface of your skin. The electrodes are placed at or near nerves where the pain is located or at trigger points.
How does transcutaneous electrical nerve stimulation (TENS) work?
There are two theories about how transcutaneous electrical nerve stimulation (TENS) works. One theory is that the electric current stimulates nerve cells that block the transmission of pain signals, modifying your perception of pain. The other theory is that nerve stimulation raises the level of endorphins, which are the body’s natural pain-killing chemical. The endorphins then block the perception of pain.
What is transcutaneous electrical nerve stimulation (TENS) therapy used to treat?
TENS therapy has been used or is being studied to relieve both chronic (long lasting) and acute (short-term) pain. Some of the most common conditions for which TENS has been used include:
- Osteoarthritis (disease of the joints).
- Fibromyalgia (aching and pain in muscles, tendons, and joints all over the body, especially along the spine.
- Tendinitis (an inflammation or irritation of a tendon).
- Bursitis (inflammation of the fluid-filled sacs that cushion joints).
- Labor pain.
- Low back pain.
- Chronic pelvic pain.
- Diabetic neuropathy (damage to the nerves that connect the brain and spinal cord to the rest of the body).
- Peripheral artery disease (“hardening of the arteries” that circulate blood to the body).
For what types of conditions and areas of the body should transcutaneous electrical nerve stimulation (TENS) therapy not be used?
Do not use TENS therapy at these specific body locations if you have any of the following conditions:
- An implantable device (cardioverter/defibrillator, neurostimulators, bone growth stimulator, indwelling blood pressure monitors). Do not use TENS therapy over or close to the areas where an electronic device is implanted. TENS could cause these devices to malfunction.
- Are pregnant. Do not apply TENS therapy to the abdomen; pelvic area; lower back; or to acupuncture points at the knee, hand or ankle. (However, TENS can be used for labor pain.)
- Cancer. Do not apply electrodes to areas of the body where there is known or suspected cancer. Do not use TENS if you have undiagnosed pain and a history of cancer in the last 5 years.
- Epilepsy. Do not apply electrodes to your head, neck or shoulders. The impulses could cause seizures.
- Deep vein thrombosis or thrombophlebitis. Do not use TENS therapy as it may increase blood circulation, which may increase the risk of dislodging a blood clot.
- A bleeding (hemorrhagic) disorder or recent or actively bleeding tissue. TENS therapy could increase bleeding at the tissue site or increase the risk of bleeding in persons with bleeding disorders.
- Heart disease. Do not apply TENS therapy to the chest if you have heart disease, heart failure or arrhythmias.
In addition, TENS should not be applied:
- To infected tissues, wounds due to osteomyelitis or if you have tuberculosis. TENS therapy may result in the spread of infections.
- To areas of tissue that have been recently treated with radiation.
- To damaged skin (Except for open wounds where the intent is to use electrical stimulation to heal tissue. In these cases, therapy should be guided by a skilled therapist.)
- Near or over eyes or mouth, front or side of neck, or on the head.
- Near reproductive organs or genitals.
- To areas of the body that lack or have reduced sensation.
- In persons who have trouble communicating or who have mental impairment and cannot provide feedback to ensure the safe use of TENS.
Electrical muscle stimulation (EMS), also known as neuromuscular electrical stimulation (NMES) or electromyostimulation, is the elicitation of muscle contraction using electric impulses. EMS has received an increasing amount of attention in the last few years for many reasons: it can be utilized as a strength training tool for healthy subjects and athletes; it could be used as a rehabilitation and preventive tool for partially or totally immobilized patients; it could be utilized as a testing tool for evaluating the neural and/or muscular function in vivo; it could be used as a post-exercise recovery tool for athletes. The impulses are generated by a device and are delivered through electrodes on the skin near to the muscles being stimulated. The electrodes are generally pads that adhere to the skin. The impulses mimic the action potential that comes from the central nervous system , causing the muscles to contract. The use of EMS has been cited by sports scientists as a complementary technique for sports training, and published research is available on the results obtained.In the United States, EMS devices are regulated by the U.S. Food and Drug Administration (FDA).
Electrical muscle stimulation can be used as a training, therapeutic, or cosmetic tool.
Physical rehabilitation
In medicine, EMS is used for rehabilitation purposes, for instance in physical therapy in the prevention muscle atrophy due to inactivity or neuromuscular imbalance, which can occur for example after musculoskeletal injuries (damage to bones, joints, muscles, ligaments and tendons). This is distinct from transcutaneous electrical nerve stimulation (TENS), in which an electric current is used for pain therapy. In the case of TENS, the current is usually sub-threshold, meaning that a muscle contraction is not observed.
During EMS training, a set of complementary muscle groups (e.g., biceps and triceps) are often targeted in alternating fashion, for specific training goals, such as improving the ability to reach for an item.
What is ultrasound?

Ultrasound is commonly used to treat soft tissue problems such as ligament sprains or muscle tears.
